See the World Brighter, Look More Refreshed

Understanding Upper and Lower Blepharoplasty

Our eyes are often the first feature people notice, and unfortunately, they are also one of the first areas to show signs of aging. Excess eyelid skin, under-eye bags, and drooping eyelids can make us appear tired, older than we feel, or even interfere with our vision. Fortunately, modern eyelid surgery offers highly effective solutions for restoring a more youthful, refreshed appearance.

Upper Blepharoplasty: More Than a Cosmetic Procedure

As we age, the skin of the upper eyelids gradually loses elasticity. This can lead to excess skin that hangs over the eyelid, creating a tired appearance and, in some cases, impairing vision.

Upper blepharoplasty is a surgical procedure that removes excess eyelid skin and, when necessary, addresses underlying tissue to create a smoother, more youthful eyelid contour. For patients whose eyelids have become noticeably heavy, the procedure can open the eyes, improve appearance, and make daily activities such as reading and driving more comfortable.

In some individuals, drooping results not only from excess skin but also from weakening of the muscles that elevate the eyelid. In these cases, a procedure to lift the eyelid itself may be recommended in combination with blepharoplasty for optimal results.

An important benefit of upper eyelid surgery is that it is not always considered purely cosmetic. When drooping upper eyelids obstruct peripheral vision and interfere with daily activities, insurance may cover the procedure following appropriate evaluation and testing.

Lower Eyelid Bags: Why They Occur

Many people are frustrated by persistent “bags” under their eyes that make them look tired even when they are well-rested.

Lower eyelid bags develop when orbital fat, which normally cushions the eye, gradually shifts forward due to weakening or thinning of the orbital septum. The orbital septum is a thin fibrous membrane that serves as a barrier between the eyelids and the eye socket. As this structure weakens with age, the fat becomes more visible, creating the puffy appearance associated with under-eye bags.

Unfortunately, creams and topical treatments can do little to correct this structural change.

Lower Blepharoplasty: A Lasting Solution

Lower blepharoplasty is one of the most effective ways to reduce or eliminate under-eye bags and restore a smoother transition between the lower eyelid and cheek.

In many cases, the procedure is performed through a small incision located inside the eyelid. Because the incision is hidden internally, there is no visible external scarring, and the approach typically does not require sutures.

During surgery, the protruding fat can be carefully removed, repositioned, or redistributed to create a more natural, youthful contour. The result is a rested appearance without looking “operated on.”

Enhancing Results with Skin Rejuvenation

While lower blepharoplasty effectively addresses the underlying fat causing the bags, many patients also have concerns about fine lines, skin texture, and wrinkles around the eyes.

For this reason, many patients elect to combine their surgery with:

  • Laser skin resurfacing
  • Radiofrequency microneedling

Both treatments stimulate collagen production, helping tighten the skin, smooth wrinkles, and improve overall skin quality. Combining these procedures often provides a more complete rejuvenation and enhances the final cosmetic outcome.
