There is certain information that we will need from you in order to personalize your consultation. To expedite the check-in process, you can access the Patient Portal and register at your convenience.

Information you may find helpful:

• You will receive a reminder via text, email or phone call 2 days before your appointment
• Please arrive a few minutes prior to your scheduled appointment time
• You will sign a Privacy Policy and Financial Agreement form upon arrival to the office.
• You can wear your normal clothes, eyeglasses, contact lenses, etc.
• During your office visit, a Cole Aesthetic Center staff member will be taking photographs of you for your medical record. A consent form must be signed for photography usage.
• If you are scheduling a cosmetic consultation, there will be a consult fee payable at the time of your appointment that will later be applied to any procedures you choose to have performed.
• We accept Visa, MasterCard, cash and personal checks
• For your convenience, we have financing options available
